About Me
My name is Emma Hazelton, and I am an Occupational Therapist with over 17 years of clinical experience. I am based in Cleveland in the Redlands, providing NDIS Functional Capacity Assessments across Brisbane and surrounding areas.

My practice focuses on NDIS report writing for complex cases involving children and adults, including Functional Capacity Assessments, SIL assessments, access reports, and documentation for ART matters.

I have a detailed understanding of NDIS legislation, NDIA decision-making processes, and current funding frameworks. My reports are written to clearly document functional impact and support needs against the Reasonable and Necessary criteria, particularly in cases involving high supervision needs, significant support worker hours, or structured supports such as adult day programs.

I offer in-person assessments across the Redlands, Brisbane, and South East Queensland, as well as telehealth assessments Australia-wide. Referrals are welcome directly by phone or email.

Services Offered
Functional Capacity Assessments (FCA) - Detailed reports for plan reviews, change of circumstance, and NDIS access requests — written to clearly document functional impact and justify support needs against current NDIS legislative criteria.

Assessments for High-Needs Children - FCAs for children with higher support needs, with a focus on evidencing the support worker hours required.

Supported Independent Living (SIL) Reports - Functional assessments documenting the level of supervision and support required for a participant to live as independently as possible.
​
Administrative Review Tribunal (ART) Reports - Standalone reports for NDIS funding disputes — structured to directly address NDIA reasoning with contradicting evidence and legislative reference.

Addenda Reports - adding a condition to an existing NDIS plan for funding acceptance, including complex presentations such as DID and FND.
​
Evidence Coordination Assistance - preparing medical and school letters - ensuring supporting documentation uses NDIS-aligned wording to strengthen the overall evidence base.

How It Works
Step 1 - Get in Touch - A brief phone or email consultation to understand the participant's diagnosis, functional challenges, the type of report needed, and any relevant deadlines. I'll confirm whether the referral is a good fit and what documentation will be needed to get started.

Step 2 - Assessment (Home visit or telehealth) - A comprehensive functional capacity assessment tailored to the participant’s presentation. Includes structured interviews, functional observation, review of existing reports and supporting documentation and relevant standardised assessments.

Step 3 - Supporting Evidence - Where required, I provide templates and guidance for GPs, schools, and specialists to ensure supporting documentation complements the report and meets NDIS requirements.
​
Step 4 - Report - A detailed NDIS-aligned report provided within an agreed timeframe, generally two weeks. The report is reviewed with the participant, family, or referrer before submission.

Step 5 - Post-Submission Support
- Where a participant does not have Support Coordination, I can assist with submitting the report and following up with the NDIA to confirm it has been received and is being considered.
For urgent or complex matters, I may continue to provide short-term follow-up to help ensure the participant's needs are clearly communicated as the matter progresses.

Pricing & Practice Scope
Fees - All reports are charged at $193 per hour in line with the NDIS price guide. The total cost depends on the report type and complexity of the case.
Typical timeframes are:

Functional Capacity Assessments (FCA): usually 14–17 hours

NDIS Access and ART Reports: generally 4–6 hours

SIL Assessments: may require additional time depending on complexity and documentation required
Additional time may apply where supporting documentation is needed, such as Home and Living or Change of Circumstance forms.
Practice Scope - My practice focuses on cognitive, behavioural, psychosocial, and neurodevelopmental presentations. I do not provide reports primarily for complex assistive technology, home modifications, or purely physical presentations.

I may be able to assist where a physical disability is accompanied by a significant cognitive or psychosocial component, or where the referral relates to an ART matter.
